Laser Skin Resurfacing In Encino: Texture, Scars And Pigment

Skin texture is what photographs and video calls reveal most unforgivingly, and it is the concern patients most often assume they simply have to live with. Acne scarring from adolescence, decades of Southern California sun, stubborn melasma, enlarged pores, dullness — all of these are texture and tone problems, and all of them respond to properly selected laser resurfacing.

Ablative, Non-Ablative And Fractional

Ablative resurfacing removes microcolumns of the epidermis and upper dermis. Because the tissue is vaporized, healing is more involved but remodeling is more dramatic — this is the approach for meaningful acne scar revision. Non-ablative resurfacing leaves the epidermis intact and heats the dermis beneath, stimulating collagen with essentially no downtime but requiring more sessions. Fractional delivery means treating a percentage of the surface area in a grid pattern, leaving untreated tissue in between to accelerate healing. Nearly everything we do is fractional, with ablative depth dialed to the goal.

The Er:YAG laser at 2940 nm is absorbed by water more strongly than any other resurfacing wavelength, which means it removes tissue with minimal residual thermal damage to the surrounding dermis. That precision is why Er:YAG produces excellent texture results with a shorter, cleaner recovery than older CO2 platforms, and why it can be used more safely in patients prone to post-inflammatory hyperpigmentation.

What We Treat

  • Atrophic acne scarring — rolling and boxcar scars respond well; deep ice-pick scars often need subcision or punch techniques alongside laser.
  • Fine lines and crepey texture, particularly perioral and periorbital.
  • Sun damage, solar lentigines and uneven pigment from years of Los Angeles exposure.
  • Melasma — treated cautiously, at low fluence, with mandatory topical suppression and strict photoprotection.
  • Enlarged pores and generalized dullness.
  • Surgical and traumatic scars, including revision of older scars.
  • Rosacea and vascular redness, using the Nd:YAG wavelength to target vessels.

Melasma And Darker Skin Tones

Melasma is a chronic, relapsing condition driven by hormones, heat and UV exposure — not merely a pigment stain to be lasered away. Aggressive resurfacing frequently makes it worse. Our approach uses low-fluence protocols, pretreatment with topical tyrosinase inhibitors, strict sun avoidance, and realistic framing: we manage melasma, we do not cure it. For Fitzpatrick IV through VI generally, conservative fluences, longer intervals between sessions, and a pretreatment regimen dramatically reduce the risk of post-inflammatory hyperpigmentation. Any provider who treats every skin type with identical settings should be avoided.

The Recovery Timeline, Honestly

For a moderate fractional Er:YAG treatment: days one and two bring significant redness, swelling and a sensation like a strong sunburn. Days three and four bring a bronzed, sandpaper texture as the microscopic treatment zones work toward the surface. Days five through seven bring flaking and peeling — do not pick. By day seven to ten most patients are wearing makeup comfortably, with residual pink that fades over two to four weeks. Lighter protocols shorten this considerably; deeper scar revision extends it.

New collagen continues to mature for three to six months, which is why the final assessment of a resurfacing result never happens at week two. Most acne scar patients need three to five sessions spaced six to eight weeks apart.

Preparation And Aftercare

  • Two to four weeks of pretreatment topicals when indicated, including pigment suppression for higher Fitzpatrick types.
  • Strict sun avoidance for two weeks before and four weeks after; SPF 30 or higher, reapplied, without exception.
  • Antiviral prophylaxis for anyone with a history of cold sores.
  • Bland cleanser and occlusive ointment during the healing phase — no actives, no exfoliation, no makeup until re-epithelialization is complete.
  • No sauna, hot yoga, chlorinated pools or intense exercise for one week.

Resurfacing rewards patience. Patients who protect their skin for a month keep results for years; patients who go to the beach at day five undo the work.

Resurfacing addresses the skin envelope. Laxity, volume and movement are separate problems requiring Fotona 4D, biostimulators or neurotoxin respectively.
